My child’s father and I want our child to have his father’s last name, do we have to go through the Court for a name change?

If the name of the father is not indicated on the birth certificate and the child has the mother’s last name, the last name can be changed to the father’s last name after paternity is established if both parents agree. You may consult with the Michigan Department of Public Health, Vital Records Division for the proper procedure
