Assessments are performed by our Case Manager who are trained specifically for senior services. Assessments can be done either in the client's home or in our office. The purpose is to identify unmet needs, to inform seniors of available services and to assist them in obtaining the needed services. An appointment may be made by calling 989 831-7476 or 616 225-7476.
